RESUMO

Objective:To explore the accuracy of artificial intelligence (AI) system based on deep learning in evaluating bone age of children with abnormal growth and development.Methods:The positive X-ray films of the left wrist of children with abnormal growth and development who were treated at the Affiliated Hospital of Guizhou Medical University from January 2020 to December 2021 were collected retrospectively. A total of 717 children were collected, including 266 males and 451 females, aged 2-18 (11±3) years. Based on Tanner Whitehouse 3 (TW 3)-RUS (radius, ulna, short bone) and TW3-Carpal (carpal bone) method, bone age was measured by 3 senior radiologists, and the mean value was taken as reference standard. The bone ages were independently evaluated by the AI system (Dr.Wise bone age prediction software) and two junior radiologists (physicians 1 and 2). The accuracy within 0.5 year, the accuracy within 1 year, the mean absolute error (MAE) and the root mean square error (RMSE) between the evaluation results and the reference standard were analyzed. Paired sample t-test was used to compare MAE between AI system and junior physicians. Intraclass correlation coefficient (ICC) was used to evaluate the consistency between AI system, junior physician and reference standard. The Bland-Altman diagram was drawn and the 95% consistency limit was calculated between AI system and reference standard. Results:For TW3-RUS bone age, compared with the reference standard, the accuracy within 0.5 year of AI system, physician 1 and physician 2 was 75.3% (540/717), 62.1% (445/717) and 66.2% (475/717), respectively. The accuracy within 1 year was 96.9% (695/717), 86.3% (619/717) and 89.1% (639/717), respectively. MAE was 0.360, 0.565 and 0.496 years, and RMSE was 0.469, 0.634 and 0.572 years, respectively. For TW3-Carpal bone age, compared with the reference standard, the accuracy within 0.5 year of AI system, physician 1 and physician 2 was 80.9% (580/717), 65.1% (467/717) and 71.7% (514/717), respectively. The accuracy within 1 year was 96.0% (688/717), 87.3% (626/717) and 90.4% (648/717), respectively. MAE was 0.330, 0.527 and 0.455 years, and RMSE was 0.458, 0.612, 0.538 years, respectively. Based on TW3-RUS and TW3-Carpal bone age, the MAE of AI system were lower than those of physician 1 and physician 2, and the differences were statistically significant ( P all<0.001). The evaluation results of AI, physician 1 and physician 2 were in good agreement with the reference standard (ICC all>0.950). The Bland-Altman analysis showed that the 95% agreement limits of AI system for assessing TW3-RUS and TW3-Carpal bone age were -0.75-1.02 years and-0.86-0.91 years, respectively. Conclusion:The accuracy of AI system in evaluating the bone age of children with abnormal growth and development is close to that of senior doctors, better than that of junior doctors, and in good agreement with senior doctors.

ObjectiveBased on the antidepressant activity, the optimal ratio of Rehmanniae Radix and Lilii Bulbus in Baihe Dihuangtang was optimized and its possible mechanism was preliminarily explored. MethodA total of 100 male mice were randomly divided into normal group, model group, Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group, and 5 different ratios of Baihe Dihuangtang group (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5 , 5∶4, 5∶3, 5∶2), fluoxetine hydrochloride group. Except for the normal group, the remaining 9 groups were treated with chronic unpredictable mild stress method (CUMS) for 21 consecutive days to establish the depression mouse model.The Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group and 5 different ratios of Baihe Dihuangtang group were administered with corresponding drugs at a dose of 2 g·kg-1 respectively, and fluoxetine hydrochloride group was administered fluoxetine hydrochloride at a dose of 0.01 g·kg-1, the normal group and the model group were respectively given an equal volume of normal saline by gavage for 21 consecutive days. The antidepressant effect of Baihe Dihuangtang with different ratios was evaluated by behavioral experiments. Western blot was used to detect the expression levels of brain-derived neurotrophic factor (BDNF) and tyrosine kinase receptor B (TrkB) in hippocampus. The level of total superoxide dismutase (SOD) in hippocampus was detected by hydroxylamine method. The level of reduced glutathione (GSH) in hippocampus was detected by spectrophotometric method. The levels of glutathione peroxidase (GPX) and glutathione S-transferase (GST) in hippocampus was detected by colorimetric method. The level of malondialdehyde (MDA) in hippocampus was detected by thiobarbituric acid method, and preliminary exploration of its antidepressant mechanism. ResultCUMS caused a significant decrease in the preference rate of sugar water and a significant prolongation of forced swimming and tail suspension immobility time (P<0.01), while the different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus were significantly reversed to varying degrees. Abnormalities of the above indicators were observed (P<0.05, P<0.01), especially when the ratio of 5∶6 had the most obvious effect on the above indicators, Moreover, CUMS caused the hippocampal BDNF and the expression of TrkB, SOD and glutathione antioxidant-related indexes were significantly decreased (P<0.01), and the levels of lipid peroxidation product MDA was significantly increased (P<0.01). However, the superior ratio of Baihe Dihuangtang (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5, 5∶4) and the single decoction of Rehmanniae Radix and Lilii Bulbus significantly reversed the abnormality of the above indicators to varying degrees (P<0.05, P<0.01). ConclusionIn summary, 5 different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus can produce antidepressant effect, especially when the ratio of 5∶6 had the most obvious effect on the above indicators, and the antidepressant effect gradually weakens with the proportion of Lilii Bulbus in the recipe decreases, and its mechanism may involve enhancing the nutritional level and antioxidant defense ability of hippocampal central nervous system.

Objective:To investigate the effect of short single reverse-α fixation of nasobiliary tube after endoscopic nasobiliary drainage.Methods:From January 2019 to October 2019, the patients who performed with endoscopic nasolbiliary drainage in Tongji University Affiliated Shanghai East Hospital were randomly divided into experimental group (short single reverse-α fixation, 155 cases) and control group (routine reverse-α fixation, 137 cases). The incidences of the fixation time, prolapse rate, nursing time, scores of nasal comfort and bile flow rate of nasobiliary duct were evaluated between the two groups.Results:The average time of nasobiliary duct fixation was (18.31±1.67) s, the prolapse rate was 5.2% (8/155) and the time of nursing was (35.03±2.68) s, which were lower than those of the control group (46.50±5.50) s, 13.9% (19/137) and (72.07±7.63) s. The difference was statistically significant ( t or χ 2 values were 60.795, 6.570, 56.629, P<0.01 or 0.05). The comfort score of the experimental group was (4.61±1.06) points, the bile flow rate was (241.52±53.95) ml/days, which were higher than (5.76±0.76) points and (174.09±47.55) ml/days of the control group, the difference was statistically significant ( t values were 10.448, 11.265, P<0.01). Conclusions:Shortening the length of nasobiliary duct combined with single reaction "α" method has the advantages of simper operation and nursing, low prolapse rate, high comfort and good drainage effect. It is worthy of clinical application.

Objective:To investigate the relationship between bone mineral density (BMD) of lumbar spine and abdominal adipose tissue in adult males.Methods:A total of 1 374 healthy male examinees in the Affiliated Hospital of Guizhou Medical University from June 2018 to September 2018 were retrospectively analyzed. The age ranged from 30 to 75 (50.9±0.30) years. Height, weight and waist circumference was measured and body mass index (BMI) was calculated. Quantitative CT (QCT) was used to measure the mean BMD value of L1 and L2. The abdominal subcutaneous fat area (SFA) and visceral fat area (VFA) at the level of L2 were measured as well. According to the BMD, the males were divided into three groups: normal group, osteopenic group and osteoporosis group. One-way ANOVA was applied to compare the age, BMI, VFA and SFA among three groups. LSD- t test was used for multiple comparisons, and Pearson correlation was used to analyze the correlation between age, BMI, VFA, SFA and lumbar BMD. Results:Totally 807 healthy males were in normal group, 451 in osteopenic group and 116 in osteoporosis group. There were significant differences in age, BMI and VFA among normal group, osteopenic group and osteoporosis group ( F=237.939, 5.788, 4.919, P<0.001, 0.003, 0.007). Age and VFA was higher in osteopenic group compared with normal group, and BMI was lower in osteopenic group compared with normal group. Age was higher in osteoporosis group compared with normal group, and the BMI was lower in osteoporosis group compared with normal group. The age was higher in osteoporosis group compared with osteopenic group. The above differences were statistically significant ( P<0.05). Although there was no statistical difference in VFA and SFA between osteopenic group and osteoporosis group, the VFA and SFA was higher in osteopenic group compared with osteoporosis group. BMD of male lumbar spine was negatively correlated with age ( r=-0.542, P<0.001), VFA( r=-0.104, P<0.001), and positively correlated with BMI ( r=0.109, P<0.001). After controlling for age and BMI, BMD was still negatively correlated with VFA ( r=-0.129, P<0.05). Conclusions:Males with abdominal obesity is more likely to manifest as low bone mass. The lumbar BMD is possibly negatively correlated with visceral adipose tissue in male.

Objective@#To detect the high mobility group A2 (HMGA2) expression in renal carcinoma, and to explore the relationship with clinicopathological features and its significance for prognosis.@*Methods@#50 renal carcinoma specimens, 50 corresponding adjacent normal kidney tissue samples, and 40 benign renal tumor specimens were used in this study. The expressions of HMGA2 mRNA and protein were detected by RT-PCR, Western blot and immunohistochemical assays, and its relationship with clinicopathological features and prognosis in the renal carcinoma patients was analyzed.@*Results@#The RT-PCR results showed that the relative expression levels of HMGA2 mRNA in the renal carcinoma, benign renal tumor tissues, and adjacent normal renal tissues were 0.84±0.23, 0.19± 0.06 and 0.08±0.04, respectively, and the expression in renal carcinoma tissue was significantly higher than those of the other 2 groups (P<0.01). The Western blot results showed that the relative expression levels of HMGA2 protein in the renal carcinoma, benign renal tumor tissues, and adjacent normal renal tissues were 0.91±0.24, 0.12±0.04 and 0.03±0.01, respectively, and the expression in renal carcinoma tissue was significantly higher than those of the other 2 groups (P<0.01). Immunohistochemical results showed that the expression of HMGA2 protein exhibited brown and tan granular, which mainly distributed in the cell nuclei. Among the 50 cases of renal carcinoma, 34 cases exhibited positive expression, with a positive rate of 68.0%. Among the 40 cases of benign tumor tissues, 3 cases had positive expression, with a positive rate of 7.5%, while among the 50 cases of adjacent normal renal tissues, there was only 1 case exhibiting positive expression of HMGA2 protein, with a positive rate of 2.0%. The protein expression of HMGA2 was significantly higher in the renal carcinoma than in the benign tumors and normal renal tissues (P=0.004). There was no statistically significant difference in the association of HMGA2 protein expressions with age, sex, tumor size and histological type (P>0.05), while significant difference did exist in the association with different statuses of TNM staging and lymph node metastasis (P<0.05). The median time to progression (TTP) in 34 HMGA2 protein-positive patients was (22.36±1.48) months and that of 16 HMGA2 protein-negative patients was (34.55±1.87) months (P<0.05).@*Conclusions@#HMGA2 plays an important role in the tumorigenesis and development of renal carcinoma, and may be used as an important predictor for estimating the prognosis of renal carcinoma. HMGA2 might become a new diagnostic and prognostic marker for renal carcinoma.

Objective To explore the CT and MR features of malignant peripheral nerve sheath tumor (MPNST) of adults.Methods The CT and MRI findings of 20 patients of MPNST confirmed by pathology were analyzed retrospectively.Results Among 20 cases,the lesions were located in the lower extremities (n=4),shoulder and spine (n=4),abdomen (n=3),chest wall (n=2),mediastinum (n=2),breast (n=1),prostate (n=1),scrotum (n=1),scalp (n=1),left maxillary sinus and orbital bottom (n=1).Nineteen cases appeared as solid masses and 1 case appeared as diffuse lesion.The maximum cross section of the lesions were about 1.41 cm× 1.42 cm-17.10 cm× 18.08 cm.Sixteen cases were ill defined margin and 4 cases were well-defined margin.CT scan showed 14 cases were lower density or isodensity and with patchy inhomogeneous density.Multiple calcification were found in 2 cases and osteolytic destruction were found in 7 cases.Enhanced CT of 10 cases showed solid component and gradually delayed enhancement,while cystic lesion and necrosis were not enhancement.The tortuous arteries were displayed in 7 cases.Seven cases were performed MRI and the lesions appeared as hypo-intensity on T1WI and hypo-intensity on T2WI with obviously high intensity of cystic component.Enhanced MRI of 3 cases showed significantly heterogeneous enhancement,cyst and necrosis had no enhancement.MR dynamic enhancement of 1 case showed time-signal intensity curve was platform type.DWI showed high signal.Conclusion MPNST has certain CT and MR characteristics which are helpful to improve the diagnostic accuracy.

OBJECTIVE@#To evaluate the effect of allergen specific immunotherapy (SIT) in patients with allergic rhinitis and asthma. METHOD A total of 68 patients with allergic rhinitis and asthma sensitized to dust mite were recruited into the study. They were randomly divided into two groups: SIT group n = 34 and symptomatic therapy (ST) group: n = 34. Patients in ST group received medication to treat, the symptoms, patients in SIT group received medication and 3 years of standardized allergen vaccine therapy. Evaluation index of therapy includes: rhinitis symptoms score, asthma symptoms score, drug score, skin prick test, serum specificity IgE (sIgE) , peripheral eosinophil (Eos) counting, lung function. The new sensitinogen rate was also assessed.@*RESULT@#Clinical symptom scores, drug scores, lung function, blood eosinophil numbers and skin test result were all improved significantly after 3-year treatment in SIT group compared to those in ST group (P < 0.01). Although the level of serum slgE was decreased,there exited no statistic diferences between two groups. Only 8.8% patients have the new sensitization in SIT group, and 52.9% in ST group. There were no serious adverse reactions in treatment process.@*CONCLUSION@#SIT for patients with AR and asthma can obtain excellent clinical efficacy.

Objective To explore the X-ray ,CT and MR manifestations of spinal giant cell tumor(GCT) for improving the diag-nosis level of this rare disease .Methods The X-ray ,CT and MR data in 16 cases of spinal GCT were retrospectively analyzed on the location ,size ,morphology ,margin ,CT density/MR signal intensity and enhancement pattern .Results 4 cases located in cervical vertebra ,8 cases in thoracic vertebra and 4 cases in lumbar vertebra ;in which ,6 cases only involved in vertebral body and other 10 cases involved in vertebral body and and appendix .The lesion affected two neighboring vertebrae in 8 cases ,and affected only one vertebra in the remaining 8 cases .16 cases were protruded into the spinal canals and oppressed the spinal cord .The prominent X-ray and CT images showed eccentric growth ,osteolytic and swelling bone destruction .MR showed isointensity or slight hypointensity on T1WI ,and isointensity or slight hyperintensity on T2WI .After intensified scanning ,the solid portion showed marked inhomoge-neous enhancement .Conclusion Spinal GCT has certain imaging characteristics .CT and MR examination could accurately display the extent of lesion and tumor involvement ,which provides more accurate image information for formulating the strategy of clinical therapy .

Objective To investigate the effect of slice orientation on the popliteomeniscal fasciculi (PMF) MR imaging and its normal MRI appearances. Methods Volumetric MRI data of 40 knees of healthy volunteers were acquired using an isotropic 3D turbo spin echo (TSE) MR sequence. The posterior tangential line to both femoral condyles was used as the reference line, and the long axis sectional images of the popliteal hiatus region were reformatted at 0°, 15°, 30°, 45°, 60°, 75° and 90° to the reference line. The MRI appearances of the PMF were scored respectively and classified. The final scores of the PMF at each slice angle were statistically analyzed by a repeated measure ANOVA. Results At 0°, 15°, 30°, 45°, 60°, 75° and 90° slice angles: scores of the anteroinferior fasciculi were (1.7±0.7), (1.8±0.6), (1.9±0.6), (2.0±0.7), (1.9±0.7), (1.8±0.8) and (1.0±0.5),respectively. Scores of the posterosuperior fasciculi were (1.5±0.7), (1.9±0.7), (2.1±0.6), (2.2±0.6),(2.2±0.6), (2.0±0.8) and (1.7±0.8), respectively. There were statistically significant differences in the average scores at each slice angle for both anteroinferior fasciculi (F = 29.744, P = 0.000) and posterosuperior fasciculi (F = 19.770,P =0.000). The anteroinferior fasciculi and the posterosuperior fasciculi had highest average scores at the angle of 45°. The percentage of type A, B and C of anteroinferior fascicali were 20.0% (8/40), 75.0% (30/40) and 5.0% (2/40), respectively. The percentage of type A, B and C of posterosuperior fasciculi were 37.5% (15/40) ,62.5% (25/40) and 0% (0/40) ,respectively. Conclusion The anteroinferior fasciculi and the posterosuperior fasciculi can be well depicted at the angle of 45° slice orientation.

Objective To discuss the value of MR T2 mapping in the research of the biomechanics and function of cartilage of knee joint.Methods Knees of 20 healthy adults before and after jogging and 19 osteoarthritis patients were examined with sagittal 8-echo SE sequence.The T2 value of cartilage was selected and calculated.The T2 values in the superficial and deep cartilage of femoral and tibial joint before and after jogging were compared,so did between the osteoarthritis patients and healthy adults.The source images were sent to the workstation to get T2 mappings.The T2 value of cartilage between before and after jogging was compared with paired-samples t test.The T2 value between superficial and deep cartilage before jogging was compared with independent-samples t test,so did between the osteoarthritis patients and healthy adults.Results The T2 values in the superficial and the deep tibial cartilage were(48.8±6.3)ms,(44.3±5.7)ms before jogging and(43.4±5.0)ms,(40.3±6.1)ms after jogging.The T2 values were significantly different between before and after jogging(t=6.004 and t=5.037,P<0.05).There was a significant difference between superficial and deep tibial cartilage before jogging(t=3.148,P<0.01).The T2 Values in the superficial and deep femoral cartilage were(52.1±5.7)ms,(47.7±5.3)ms before jogging and(47.2±4.5)ms,(43.6±4.1)ms after jogging.The T2 values were significantly different between before and after jogging(t=6.169 and t=5.957,P<0.05).There was a significant difference between superficial and deep femoral cartilage before jogging(t=3.384,P<0.01).The T2 mapping showed those changes.The mean T2 value in the tibial cartilage of osteoarthritis patients was(56.0±9.1)ms and was higher than that of healthy adults.There was a significent difference between osteoarthritis patients and healthy adults(t=-3.446,P<0.01).Conclusion T2 mapping can be used in the research of biomechanics and function of cartilage and has a prelimilary value in the diagnosis of cartilage degeneration.
